Abstract
This research work looks at Evaluation of the problems and constraints facing health record keeping and professionals. Medical record is very important for the treating doctor to properly document the management of a patient under his care. Medical record has evolved into sciences of itself. This will be the only way for the doctor to prove that the treatment was carried out properly. Moreover, it will also be an immense help in the scientific evaluation and review of patient management issues, medical records from part of the management of patients. Health records is a wide term embracing all record details incidental to all patients care in the hospital written by doctors, nurses and other paramedical staff enclose in a folder. The aims and objectives of this study were clearly stated the significant scope and limitation of the study. The literature review discussed the following health record, ways to achieved good health care delivery, principals guiding health record keeping, methods of record keeping, constraints facing health information managers. The chapter three is all about research methodology and makes us to know the type of data used, method of data collection used in carrying out the research work. One hundred and twenty (120) questionnaires were administered and one hundred and ten (110) was completely retrieved by the researcher and the result was analyzed using percentage. The chapter five discussed the summary conclusion and recommendations were based on the findings of the result. Findings revealed that a health record keeping is important in patient care and it is the primary source of data in the hospital. In conclusion, health records should be computerized patients or untrained staff should not be allowed to handle patient records.
